1. Diabetes insipidus i.e. frequent urination

2. Dehydration

3. Hyperglycaemia or high blood sugar

But if the results are normal range the resulting conditions could be overhydration or Hyponatremia that is the presence of too little sodium. 

The urine osmolality normal value range is said to be 500 to 850 mOsm/kg. Anything higher than this can lead to conditions like dehydration, high glucose or even congestive heart failure. Conditions resulting from low osmolality can be kidney failure, overhydration or renal tubular necrosis.
